LINK Healthcare is a growing and innovative healthcare solution, which provides direct primary care to employees and eligible dependents. The clinic provides a wide range of services including primary care, occupational health, acute and chronic disease management, and urgent care. We also offer a comprehensive health and wellness program that promotes employee satisfaction and integrates in unison with our clinical team. The clinic provides comprehensive lab services, x-rays and common office procedures. LINK acts as a health navigator, helping employees choose the highest quality care at the best possible price.

The health coach educator works with clients to assist in achieving health-related goals for improving the overall population health and well-being of their Team Members. They provide guidance and accountability towards making positive changes and improvements in each patient’s life including health, wellness, fitness, relationships, mental health, etc.

  • Our employer partners work closely with the LINK Team to remove any barriers to accessing high quality care
  • Being independent of any "big system" limitations, we can always direct the patient to the highest quality, low cost options available when care is needed outside the LINK continuum
Requirements:

Essential Duties and Responsibilities

  • Conducts both initial and ongoing consultations and comprehensive lifestyle assessments for each patient.
  • Provides verbal and written guidelines to reduce and/or eliminate unhealthy decisions and behaviors.
  • Leverages assessment information for analysis and formulates an individualized plan of care tailored to each patient’s identified needs.
  • Develops and conducts seminars for clients and other participants.
  • Utilizes motivational interviewing and communication skills to ensure maximum participation and engagement.
  • Provides individualized coaching to support and empower patients to make ongoing and independent decisions regarding improvement to their overall health through lifestyle changes.
  • Understands limitations and possesses ability to recognize patient needs through knowledge, education, and experience when an outside referral for ongoing care or counseling is required.
  • Possesses ability to follow established program workflow and document client data into an electronic chart in a timely manner.
  • Possesses ability to engage in face-to-face or virtual telephone consultation and treatment.
  • Has verbal and written communication skills.
  • Maintains client confidentiality and safety and displays compassion while helping others.
  • Performs other duties as needed or specified.
  • Must be willing to go on-site with employers and engage with the employees to promote health and wellness.
  • Experience in project planning and wellness challenges is desired, but not required.

  • Previous job experience with health coaching is highly desired.
  • Ability to maintain HIPAA compliance and safety provisions.

Minimum Qualifications:

  • Bachelor’s Degree or equivalent work experience in a health-related discipline such as nutrition, counseling, fitness, health education, psychology, wellness, nursing, or other health care.
